There are a variety of ways in which you can convert existing paper based forms into electronic copies capable of feeding data directly into a database and/or storing the documents within a document management system such as any of the Microsoft SharePoint suite of applications.
One cost effective way is via the development of Microsoft InfoPath forms which can accurately reflect the look and feel of the original forms and hence lessen the learning curve for migrating between the systems. There are also a multitude of ways in which you can process the data from an InfoPath form including the creation of custom workflows within SharePoint to route the form to the relevant person and track the results.

When combined with an installation of either Windows SharePoint Services or Microsoft Office SharePoint Server we can also provide a quality assurance database to let you keep track of number of forms submitted, form completeness and a variety of other metrics.
